Nurse-Family Partnership seeks to address some of the serious social problems affecting our community. Building on this research, a model called NFP Guides attempts to address the fragmented structure of the social services industry by taking a holistic, whole-family approach. NFP Guides provide expert advice and troubleshooting solutions to support NFP Nurses in connecting their participating families to quality resources in the community based upon each individual family's specific needs, strengths, and motivation, as identified by the nurse home visitor. Services might address immediate needs or short-term problems. However, the primary focus is to create long-term solutions, individual accountability, growth, and development of human potential consistent with the Nurse-Family Partnership model.
The variety of services needed within some families is greater than any one organization can manage. The NFP Guide’s effective application of these wrap-around services are achieved, in part, through existing and new collaborations with community organizations and resources capable of providing services and opportunities that best fit each individual’s context, need, and motivation.
